What Should I Bring to My First Appointment?

We will need a copy of your ID and insurance card, as well as an acceptable form of payment to keep on file.  Please also bring a list of all your current and past medications/dosages with you.

 

 What If I Experience An Emergency?

In the event of a medical/psychiatric emergency, please dial 911, go to the nearest emergency room, or call the National Suicide Prevention Line at 1 (800) 273-8255.

How Is My Privacy Protected?

Confidentiality is always respected. You are also protected by law under the Health Insurance Portability and Accountability Act (HIPAA) which contains a privacy rule that creates national standards to protect individuals’ medical records and personal health information.

 Do You Offer Telemedicine Appointments?

Yes!   Telemedicine appointments are available for new and current patients who are unable to come into the office for an appointment. While not every condition is right for a Telehealth appointment, there are a wide variety of times when a video call is appropriate and convenient, including: Depression, Anxiety, Mood Disorders, Medication Management

 

What Is A Psychiatric Mental Health Nurse Practitioner?

A Psychiatric Mental Health Nurse Practitioner (PMHNP) is an advanced practice nurse who cares for individuals with psychiatric and mental health disorders. They assist patients struggling with mental illness,  just like psychiatrists.

The main difference between the two roles is that the psychiatrist is a medical doctor and the PNP is an advanced practice nurse.   Both roles care for the same population, and in many ways, these two careers overlap.

 

What Are Your General Office Policies?
  • No-Show fee of $100 will be charged to your card on file

  • A urine screen is required for all new patients, this cost is included in the new-patient rate. 

  • A urine screen may be requested at the discretion of your provider during any medication management follow-up visit. Cost of $15 is responsibility of the patient.  

  • Payment is due for new patient appointments at the time of scheduling to guarantee the appointment.

  • Our office does not complete Disability paperwork. 

  • Any medication refill requests will be considered on a case-by-case basis and if approved by the provider, a fee of $25 will be applied if refilled without an appointment.
